The third stage of therapy is the maintenance step. It is a stage that begins as soon as the patient leaves the treatment facility and goes back home. It usually lasts for months or years. The patient will from this point onward be seen on an outpatient basis during scheduled appointments. The therapist uses such appointments to assess for progresses and to introduce new treatments if need be.

Although drugs may be prescribed in some cases, behavioral therapy is the main form of management during the maintenance stage. The therapist will use various strategies to discourage relapse. Both negative and positive reinforcement are used. Negative reinforcement is where undesirable behavioral patterns are rebuked while positive reinforcement refers to the promotion of desirable behavioral patterns.
